Website information and medical records
NuSelf Bariatrics is a practice of Sashidhar V. Ganta, M.D., P.A. This policy concerns use of the public website. Our Notice of Privacy Practices addresses protected health information used in patient care. The two documents serve different purposes. Information used to deliver the website
Hosting and security providers may process technical information such as IP address, browser information, requested pages and access logs to deliver and protect the website. Access to this private review site is controlled by its hosting service. Public educational pages do not ask you to enter a medical history, insurance number or payment information.
Assessment and secure patient communication
In this review version, assessment answers remain in memory only while the assessment is open, are cleared when it closes and are not submitted to NuSelf. No personalized analysis is produced. Secure patient forms and account access require the practice-approved portal. Do not place sensitive information in public URLs, social media or ordinary website feedback.
Videos, maps and external links
Videos load from YouTube only after you choose to play them. That service may receive technical information and apply its own privacy practices. Map, social media and sister-practice links take you to separate services. Locally hosted fonts and images do not require a request to a font provider. Review the policies of external services before sharing information with them.
Your questions and choices
You can browse educational content without completing the assessment or playing videos. Contact the Office Manager to ask about information associated with your interaction, request assistance or raise a privacy concern. Requests involving medical records follow the process described in the Notice of Privacy Practices. This policy will be updated when website services or data practices change.
