Before your first visit
Have your identification, insurance card if applicable, medication and supplement list, relevant diagnoses and previous treatment history ready. For a revision or second opinion, request the original operative report, relevant imaging or endoscopy and recent laboratory results. The team will explain which forms apply to your appointment.

Requesting your records
